Next week, more than 40,000 healthcare professionals will descend on Orlando for the 2017 HIMSS conference. This is one of our favorite weeks of the year, where we get to talk with smart, passionate people about how they’re driving better patient care. We’ll be showcasing EXTEND, a tool that rallies the entire care team around patients undergoing joint replacement surgery. It prepares patients and their families for surgery by guiding them through educational videos, surveys, and activities. Following surgery, physicians, nurses, and care coordinators can follow their recovery and communicate with the most high-risk patients. It’s helping to save time, reduce surgery cancellations, and eliminate unnecessary follow-up visits.
You can also learn about DECIDE, a population health tool that helps people self-manage their diabetes. DECIDE helps patients identify and overcome barriers to living a healthy lifestyle, especially around nutrition, physical activity, and stress levels. The literacy-adapted program guides patients through a series of educational modules to learn about healthy behaviors and how to adopt them in their own lives. Providers can easily monitor their progress and intervene, when necessary. The program has been shown to improve A1C levels, lower blood pressure and cholesterol, and improve medication adherence.
